I put perfume on 2 days ago and now it looks like the picture it’s very red , when I touch it hurts and swollen I’ve been cleaning it with a saline water and covered after but can’t see any changes! Do you think I should remove it ( but scared if hurts ) or just keep doing the same
I’ve never heard of a dermal piercing, but yes, obviously you should take it out, in just the same way that you would remove a splinter or thorn. I can’t see how that is ever going to heal. Really not the same as an ear piercing!
Taking it out is bad advice for piercings generally as the surface can heal over trapping an infection inside, I don't know if the same is true for dermal piercings.

Please don't pour hydrogen peroxide on it and please don't take the advice of anyone who has never seen a dermal piercing before and simply can't understand how they could possibly heal - that would be like learning to ride a bike from someone who has never seen a bicycle before.
I'd go back to your piercer for advice as soon as possible, assuming you went to someone reputable and experienced. If you didn't then go to someone reputable and experienced now - I'd see if there's anyone near you from this list: https://www.ukapp.org.uk/members-listing/. Don't remove it without speaking to them first, especially if you're not confident to do so.
Were you told to use saline? I got a daith piercing last summer and was told the advice has changed since my last piercings several years ago and just to clean it with unscented soap and water when I showered and otherwise leave it alone and not to use saline on it - it's healed the best of any of my piercings.
Does My Dermal Look Infected?
Currently have two dermals in similar position to you. Unless you can feel the metal or you have a lump developing I would keep it clean and covered and wait it out.
Mine reacts with certain perfumes and gets inflamed when it's knocked /pushed. Recently had an infection with a puss lump and after keeping it clean/popping the infection and covering it it's completely fine and settled and still in place where it should be.
Dermals are very finicky just gotta look after it. If you see any sign of rejection and feel the metal where it shouldn't be go to your piercer ASAP. you know your body better than I do

I might not know about this sort of piercing but I do know that trying to stick things in n yourself is likely to be a problem, and not the same as piercing through a thin fold of skin where a tube of skin can develop around the piercing.
I agree with some of the other posters, speak to your piercer first. It may well just be very irritated by the perfume rather than infected and if you remove it then any infection will be trapped in as a pp mentioned. I haven't had a dermal piercing but I have had a surface piercing, which I removed as it was rejecting. It had been in for 18 months but closed up incredibly quickly. The APP linked above is a great place to start if your piercer can't help.
